The route was designated in 1942 between Santo and U.S. Highway 80, replacing Spur 40[2]. The route was extended north to U.S. Highway 180 by 1945, replacing FM 130. The route was extended southeast to Granbury by 1949, replacing FM 7. By 1953, the route had been extended north to a road intersection south of the Brazos River, replacing FM 1193. By 1956, the route had been extended north across the river to Graford, replacing FM 1194. The current route was completed in 1985 when it was extended north to Jacksboro, replacing a former route of FM 206[3] and also extended south to then U.S. Highway 81 (now SH 81) at Grandview, replacing Farm to Market Road 208[4] and a former route of FM 110.[5]
FM 208 was designated by 1946 from Granbury via Acton to US 377 northeast of Granbury. In 1985, the portion from Acton to US 377 was transferred to FM 167. It was rerouted over an old route of FM 1192 to Cleburne[6]. 2 months later, FM 208 became part of FM 4.
The Honda Sport Touring Association has named the section between Palo Pinto and Granbury, through the Palo Pinto Mountains, one of Texas' top ten roads for riding enjoyment and scenery.[7]
